Dormitory Check-out Request

Please indicate your intention to move out of the dormitory by filling out the check-out request form.

Completing the form is mandatory and is a prerequisite for starting the check-out process.

Uploading a document confirming payment of the dormitory fee is required for approval of the request.

After successful verification, a Check-out Form will be issued confirming that the student has no outstanding dormitory debts.
